Audit Survey Form Filling

Nature of job: Form Filling Project.

Data Entry Mode: Through Given Software by client.

Payment Security: Payment Guarantee through LC/Bank Guarantee (One month billing amount).

Eligibility: At least one year experience on data-entry/form-filling.

Turn Around Time: 22 Working days per month.

Duration of project: 15 months initially.

Work Flow: Minimum 4000 forms per month/ per seat.

Minimum no. of Slot Booking: 10pc/one slot.

Rate per Form: $1 for each 5 form/Rs.10.87 per form (approx).

Billing Cycle: Every 15 days.

Workload Distribution: Data will be provided to the user on monthly basis through FTP server only.

Signup: Online/Onsite.

Terms and Conditions:
• LC/BG will be provided only after booking above 2 slots.
• Direct client sign-up (USA) with centers.
• No upfront, only10% royalty of billing amount is considered as consultancy charge.
• LC/BG charges should be deposited by center itself.
• One month billing should be deposited on Direct USA company account directly as security deposit, what will be fully refundable after the termination of agreement.
• Compensation will be 35% (In case of non-availability of data or, delay of payment; this would be mentioned clearly in the agreement) of billing amount.
